Important Instructions Before Filling the Enrollment Form

Welcome to Saint Joseph's Academy Foundation — a dedicated platform committed to helping students prepare for government competitive examinations through free and affordable education. We are pleased to have you join us on your journey toward a brighter future.

Before you begin filling out the enrollment form, please ensure that you have the following information and documents ready.

  1. Full Name of the Applicant:
    Enter the full name exactly as mentioned on the Aadhaar Card.
  2. Parent / Guardian Name:
    Enter the full name of the parent or guardian as per official/legal documents.
  3. Date of Birth:
    The applicant must be between 16 and 40 years of age to be eligible for enrollment.
  4. Email Address:
    Provide a valid and active email address of the applicant. You will need to verify it using an OTP sent to the email.
  5. Mobile Number:
    Enter a valid 10-digit mobile number of the applicant. Duplicate numbers are not allowed.
  6. WhatsApp Number:
    Provide the applicant’s WhatsApp number and verify it using OTP. This number will be used for important communication.
  7. Parent / Guardian Mobile Number:
    Provide a valid contact number of the parent or guardian for communication purposes.
  8. Residential Address:
    Enter the complete residential address including House/Building Name, Street/Locality, PIN Code, Area, City, District, State and Country.
  9. Passport-Style Photograph:
    The photograph must meet the following requirements:
    • Format: JPG, JPEG, or PNG
    • Dimensions: 400 × 500 pixels
    • File Size: 20 KB to 100 KB
  10. Aadhaar Card (PDF):
    Upload a combined PDF of the front and back sides of the Aadhaar card.
    • Format: PDF
    • File Size: 100 KB to 200 KB
  11. UDID / Disability Certificate (If Applicable):
    Applicants declaring disability must upload a valid UDID or disability certificate.
    • Format: PDF
    • File Size: 100 KB to 200 KB
  12. Language Requirement:
    The entire enrollment form must be filled only in English. Do not use Hindi or any other regional language while entering information in the form fields.

Important Notice:
Duplicate submissions using the same Email Address, Mobile Number, or WhatsApp Number are not allowed. Please ensure that all information provided is accurate and belongs to the applicant.

Tip: Upload a recent, front-facing color passport photo (400 x 500 px). Use JPG, JPEG, or PNG format. File size must be between 20 KB and 100 KB. Ensure the image is clear with a neutral expression and a plain, light background.
Tip: Upload applicant's Aadhaar card as a single PDF file (Minimum 100 KB & Maximum 200 KB).
